The Specialised Supported Living Service helps adults with learning disabilities, physical disabilities and complex needs across Leeds to live independently.
Their support includes:
- direct support in people’s homes
- personalised packages of care and support
- help to live safely and healthily in own accommodation
All referrals should be made via the ‘Leeds Adult Social Care Learning Disability Nominations Panel’. This can only be done by a social worker or care manager.
