Urgent referrals to the Crisis Assessment Service (CAS)

The Leeds Crisis Assessment Service (CAS) is a city-wide acute mental health service run by Leeds and York Partnership Foundation Trust (LYPFT).

The CAS offers assessment to people, 18 years and over, who are experiencing acute mental health problems that may pose a risk to themselves and/or others and require an assessment that day or within the next 72 hours.

Referrals are made by professionals via the Single Point of Access.

Care Homes Team – supports people with mental health problems who live in care homes across Leeds.

Community Mental Health Services (CMHS) – there is a MindWell page you can share with patients who are being referred to CMHS to help them understand what to expect at their assessment appointment.

Admiral Nurse Team, the St James’s University Hospital Transition of Care Admiral Nurse service is available to people with dementia who are inpatients at St James’s University Hospital.
These nurses offer appointments for people who care for someone with dementia who has complex needs. Email the completed referral form to admiralnurseteam.lypft@nhs.net

Adult Attention Deficit Hyperactivity Disorder (ADHD) Service

Aspire (Leeds Early Intervention in Psychosis Service) – you can also refer directly to the service through an Aspire referral form on the Community Links website.

Autism Diagnostic Service (LADS)

Community Learning Disability Teams (CLDT)

CONNECT: The West Yorkshire and Harrogate Adult Eating Disorders Service – referrals can be made by GPs or other mental health professionals. The service criteria is for people with moderate to severe Anorexia Nervosa (AN) or Severe Bulimia Nervosa (BN), OR less severe/ atypical eating disorders but the presence of an additional risk factor (such as diabetes, pregnancy, related physical health risks). In addition, the service has an early intervention pathway – FREED; for individuals aged 18-25 with onset of less than 3 years, CONNECT accepts referrals for mild AN and mild/ moderate BN. CONNECT also has a dual diagnosis pathway within the service, for individuals who meet the service criteria, but also experience drug or alcohol dependency (run in partnership with Forward Leeds). CONNECT also runs a monthly carers group.

EMERGE Leeds: Complex Emotional Needs Service, formally called the Personality Disorder Managed Clinical Network (Leeds). Referrals can be made to all parts of this service by a professional working with the service user. Referrals to the Journey programme or Dialectical Behaviour Therapy (DBT) skills groups can be made by a professional or by self-referral. You don’t need (or have to agree with) a diagnosis of ‘personality disorder’ to access the service.

NHS Northern Gambling Service offers specialist therapy to people affected by gambling addiction including family, partners and carers. The service is based in Leeds City Council’s Community Hub at Merrion House and covers the whole of the North of England.

Leeds Gender Identity Service offers assessment and support to people aged 17 and above with Gender Dysphoria.

Leeds Recovery College offers free educational courses that focus on keeping people in Leeds mentally and physically well. The aim of the courses is to help people learn more about mental health, work out what keeps them and others well, and find ways to enjoy life more. Courses are open to anyone from across the city who would like to better look after their own wellbeing. Referral is not necessary and people can enrol themeselves on courses. Perinatal Mental Health Service – referrals of women experiencing significant mental health difficulties during pregnancy and the first year following a child’s birth can be made directly to the specialist Perinatal Mental Health Service.
